Getting Started All you need to begin your program at LVPT is a prescription from a physician. To make your first appointment, call our office and we will schedule you within the next few days. On your first visit, we request that you arrive about 15 minutes early with your insurance card(s) and prescription. You will need to complete intake paperwork before you begin. For an effective rehabilitation program, it is important that you make and keep all of the appointments necessary for your treatment. Your therapist will tell you how often you need to come in. If you cannot make an appointment, please call our office immediately to reschedule.
